A CASE REPORT OF SKIN CANCER OF THE PALM UNDERWENT RADICAL SURGERY COMBINED WITH PEDICLED FLAP COVERAGE AT THAI NGUYEN NATIONAL HOSPITAL

Abstract:
Squamous cell carcinoma is a common type of skin cancer. In the hand
area, this is the most common type; the most common location is the
dorsum of the hand. The hand area has complex anatomical characteristics
from ligaments, muscles, nerves, skin, and blood vessels, so treatment is
complex. Due to the complexity of the treatment, this article aimed to
describe the first case of skin cancer in the palm area that was treated with
radical surgery and a pedicled flap at the Oncology Center, Thai Nguyen
National Hospital. A 63-year-old male patient was admitted to the hospital
with a 2x3 cm tumor in the right palm; the tumor was ulcerated, secreted a
foul-smelling fluid, had limited mobility, was on an old surgical scar, and
invaded the surrounding skin tissue. The patient had a tumor biopsy and
was diagnosed with Squamous cell carcinoma T4N0M0. The patient
underwent wide excision of the tumor in the right-hand area, right axillary
lymph node dissection, and peripheral radial forearm flap. After surgery,
the disease progression was stable. With a diagnosis of T4N0M0 stage II
squamous cell skin cancer in the right-hand area, it is classified as having a
very high risk of recurrence. The patient was scheduled for postoperative
radiotherapy.
