A systematic review of the psychometric properties of instruments to measure Alcohol use among adults

Overview

Abstract:

Background: Alcohol is widely used in the world and can lead to many undesirable consequences. Measuring an individual’s alcohol use is a critical task for healthcare professionals. It is important to accurately identify alcohol use patterns, as different interventions are provided for each drinking pattern. However, the synthesis of psychometric properties of alcohol use measures is still limited. Objectives: To systematically report the psychometric properties of instruments used to measure alcohol use. Methods: PubMed, Scopus, Science Direct, and WILEY online library were searched. The selection was limited to studies written in English and published between 2014 and 2023, conducted among the adult population, and presented the process of evaluating cultural validations and adaptations of alcohol use instruments. The researcher used the COSMIN Checklist to evaluate the methodological quality. A narrative synthesis was used to summarize the findings. Results: A total of 7309 records were identified, of which 15 studies were included. Five instruments were identified. Among these, two instruments aim to measure alcohol use only, while the others measure substance use including alcohol. The most tested instrument was the Alcohol Use Disorders Identification Test (AUDIT). The construct validity, internal consistency, and criterion validity are tested and reported clearly. However, the content validity, inter-rater, intra-rater reliability, cross-culture validity, and responsiveness were less likely to be tested. Conclusion: Using AUDIT to measure alcohol use is highly recommended. Although other instruments are widely used, some psychometric properties require further examination.
