OUTCOMES OF POSTOPERATIVE CARE IN PATIENTS UNDERGOING PERCUTANEOUS GASTROSTOMY UNDER LOCAL ANESTHESIA AT CENTER FOR ONCOLOGY OF THAI NGUYEN NATIONAL HOSPITAL

Abstract:
Objective: To evaluate the outcomes of postoperative care in patients undergoing conventional percutaneous gastrostomy under local anesthesia at Center for Oncology of Thai Nguyen National Hospital.
Subjects and Methods: A prospective, descriptive cross-sectional study was conducted on 58 patients who underwent conventional percutaneous gastrostomy under local anesthesia, fulfilling defined inclusion and exclusion criteria, from January 2024 to September 2024. Results: The mean age was 54.4 ± 9.6 years, with the 60–69 age group accounting for the highest proportion (41.4%). The majority of patients were male (96.6%). Esophageal cancer was the most common indication for gastrostomy, comprising 58.6% of cases. 62.1% of the patients were identified as being underweight. The most frequent postoperative complication was wound infection, occurring in 6 out of 58 patients (10.3%). Gastric leakage around the feeding tube was observed in 2 patients (3.4%). All patients received standardized postoperative care, including wound dressing change, nutritional counseling, via-sonde feeding instructions, complication monitoring guidance, and home care education. By 7-day postoperation, 84.5% of patients reported discomfort related to the gastrostomy tube; nevertheless, all patients expressed satisfaction with the procedure and found feeding via the tube feasible and manageable.
Conclusion: Percutaneous gastrostomy under local anesthesia via the left subcostal approach is an appropriate and well-tolerated technique for establishing enteral access in cancer patients. Wound infection was the most prevalent complication. All patients received comprehensive care and were satisfied with percutaneous gastrostomy.
